To assist us in responding to individual requirements, please inform the College of any special needs your child has which may affect his/her learning, participation or welfare during school hours by completing this form: Student Individual Needs and Medical Form.

If a student is required to take prescribed medication during the day, either temporarily or ongoing, the parent/caregiver needs to inform the College by completing the following form: ​
